IHSAA PHYSICAL REQUIRED TO TRYOUT: DETAILS HERE

Woodside Parents, 

Hello! Coach Paige, Becky, and I hope everyone is having a wonderful summer.  We want to take a moment to communicate all of the summer and fall volleyball info:

#1 MOST IMPORTANT ITEM ---> IHSAA Physicals are required to try out ---> More Details Below...

If your daughter plans on trying out for volleyball in the fall, please make sure they have the REQUIRED 2026-2027 IHSAA Physical forms, Concussion form, and Drug Testing form completed before the 1st day of tryouts.  Please DO NOT wait and put this off until the last moment, get it scheduled ASAP!  

Where can I find WMS volleyball information?

Below are several opportunities to get in the gym before tryouts. . . 

Summer Camps:

 Here is a link to all of the local and some not so local volleyball camps/clinics in the area: SACS Camps.  

Woodside Open Gyms: 

July 14, 16, 28, 30: 8-10am at the WMS gym, Door 14. Four opportunities to knock off the rust, learn, improve, and get comfortable in the gym. Attend as many as possible! No registration required. 

July 21-23: Homestead High School will be hosting a "MS Try-Out Prep Camp"

Monday Aug. 3rd: 10th Annual SMS+WMS Pre-Season Volleyball Clinic, instructed by University of Saint Francis head coach, Scott Haddix. WMS Door 14.

WMS Tryouts: WMS, Door 14

Tuesday, Aug 4: 2:45-5pm

Thursday, Aug 6: 2:45-5pm

First Practice: Friday Aug 7: 2:45-5pm with Parent Meeting directly following at 5pm.

6th Grade Girls Fall Camp: Scheduled to start after Fall Break.  Please spread the word to any 6th graders you may know.  Details will be available after Labor Day.

SPORTS PHYSICAL DETAILS:

SMS, WMS, and HHS use the same system, FINAL FORMS for IHSAA Physicals and other documents.

Summary: At the end of this process, you will need a IHSAA physical completed and signed by a physician.  You will take it and upload it into FinalForms.  So, this means you can create the FinalForms accounts first, or you can get a physical completed first, either way works.  BUT the first time you walk through the FinalForms website and register yourself and your athlete it does take some time.  It is not a quick process.  This is a 100% online process now.  The Woodside MS front office will NOT accept any paperwork.  Everything must be uploaded and approved in Final Forms before your volleyball player can tryout.

FINAL FORMS

Final Forms is a three-step process...

  1. Parent Registration: Create an account, or login if you already have one.  Follow this all the way through including confirming your account via email.

  2. Register your Student: Login under the Parent Icon, Click Add Student.  This is a very lengthy process that will include many forms/screens.  Follow this all the way through including confirming your account via email.  The email used for the student must be a personal student email address or a parent email address.  SACS does NOT give middle school students email addresses. 

  3. Upload your completed physical that has been signed by a physician.  Scan or take a picture of your completed physical that has been signed by a physician, upload it into Final Forms, and follow it all the way through.
