ATTENTION COACHES, ATHLETES & PARENTS
As plans are made for summer camps, summer workout routines and the 2022-23 athletic season, NEW IHSAA Pre-Participation forms are required and must be dated by a medical professional after April 1, 2022. Updated forms are now available in the athletic office or for download by CLICKING HERE.
NOTE: Both pages of the Physical History Form are now required to be kept at the examining medical professional's office and not the Athletic Department.
IHSAA forms that are turned in on the 1st day of the activity will not be processed that day. Plan accordingly. These forms must be turned into the athletic office or the coach - days prior to the activity. Forms may also be submitted at the Door 1 reception office during the summer (7:30 am-2:30 pm). All forms must be complete with signatures, medical professional’s license number, insurance information, dates, etc.
Remember – if you want to play – don’t delay!
**Forms must be filled out in their entirety and delivered to the Athletic Department BEFORE any participation can occur**
1. Forms can be delivered to the Athletic Office in the Main OFFICE area. Enter through DOOR 1. NOTE: During the summer months - please call 431-2701 for summer office hours.
2. Scan and email forms to [email protected] (Woodside)

REMEMBER:
Physicals must be performed after 4/1/22. Make sure to fill out every page completely - including parent/student signatures, insurance provider and member/policy ID#, and student info. The medical professional who performs the physical must also sign the form (stamps are not allowed) and include their address, phone, license # AND date of exam.
IHSAA Rule 3-10 requires that "Between April 1 and student’s first Practice in preparation for interschool athletic participation, the student shall have a physical examination by, or shall provide certification from, a physician holding an unlimited license to practice medicine, a nurse practitioner or a physician assistant, who shall clear the student for athletic participation using the current IHSAA PreāParticipation Evaluation form."
Walk-in physicals are available at the MinuteClinic inside CVS at Getz and Illinois Road.
- The physician's (PA or LPN) signature must be hand-written. NO signature stamps will be accepted!
- The physician's (PA or LPN) signature AND license number must be on page 2.
- If we receive an IHSAA physical with a stamped doctor's signature, it will not be accepted and considered invalid.
Parents and students must sign at the bottom of page 1 and on page 4 of the physical.
